Advanced Nurse Practitioner

Advanced Nurse Practitioners – Visiting – Kidderminster Medical Centre

Working at our Kidderminster Medical Centre, we are looking to recruit an Advanced Nurse Practitioner – Visiting, will be an experienced nurse who, acting within their professional boundaries, will provide care for patients presenting at the practice from initial history taking, clinical assessment, diagnosis, treatment and evaluation of care. They will demonstrate safe, clinical decision-making and expert care, including assessment and diagnostic skills, for patients within general practice.  Preference for the post holder to be NMC registered independent prescriber. Working 30 hours per week.

They will work collaboratively with the general practice team to meet the needs of patients, supporting the delivery of policy and procedures.  As part of the Wyre Forest Health Partnership, the role will be based at Kidderminster Medical Centre, but the post holder may be required to work at other WFHP sites. Healthcare Assistant

Healthcare Assistant – Kidderminster Medical Centre

Kidderminster Medical Centre are looking to recruit a qualified Healthcare Assistant who will assist the practice clinical team in the provision and delivery of prescribed programmes of patient care, strictly in accordance with specific partnership guidelines and protocols, working 29 hours per week.  The role will include:

  • New patient health checks
  • ECG recording
  • Phlebotomy
  • Injections to include Influenza, Pneumococcal, Vitamin B12, Shingles
  • Processing and management of laboratory samples requested by GPs/nurses
  • Vaccine/cold chain storage, monitoring and recording
  • Chaperoning duties
  • Clearing and re-stocking consulting rooms
  • Preparing and maintaining environments and equipment before, during and after patient care interventions including assisting GPs during the performance of minor operations where applicable

Practice Nurse – Bewdley Medical Centre

Working at our Bewdley Medical Centre, we are looking to recruit a Practice Nurse, working 25 hours per week. To undertake a range of nursing assessments and provide appropriate care / treatment in conjunction with GPs according to practice policy, protocols etc. Strive to improve standards of care and be involved in identifying practice population needs. Chronic Disease management is an essential part of the role as well as the mentoring of our Health Care Assistants. Knowledge of the GMS contract, e.g. The Quality and Outcomes Framework (QOF)  and an understanding of LES/DES is desirable.

First Contact Physiotherapy Practitioner

First Contact Physiotherapist – Wyre Forest Health Partnership

The Partnership is looking to recruit an extended Scope, First Contact Physiotherapist Practitioner. Reporting to our Lead Physiotherapist Practitioner, the role will involve working autonomously, across the 5 Partnership sites, providing highly specialised assessment and treatment of patients within your area of clinical specialty. As such you will advise, influence, promote and create models of practice, which offer a continuum of care and equality of access.

This role is suitable for a Physiotherapist with enhanced skills so that they can be the first point of contact for patients with MSK symptoms. Applicants must have current UK professional registration. Experience in a similar role within Primary Care is desirable. The ability to carry out joint injections or willing to train is essential.

This post is a stand-alone role but you will report to the Lead Physiotherapist Practitioner who will be able to offer support and guidance to deliver care within the partnership setting.
