Data Administrator

Wyre Forest Health Partnership: one successful and ambitious health partnership working at scale together to provide high quality innovative patient care, delivered by a valued and respected team.

The post-holder will be responsible for the review and update of patient data across the partnership.  Key responsibilities will include working alongside the Data Team, Transformation Manager, Operations and Quality Manager and Clinical Leads.  To include:

·       Adhere to our core principles: Care, Commitment and Integrity.

·       Provide outstanding patient service by ensuring patients are called in a timely fashion for any care, reviews or treatments that are due.

·       Co-ordinate the patient recall process for annual reviews, health checks, minor operations and any other routine treatment needed.

·       Liaise with all sites to ensure full capacity appointment booking for reviews.

·       Request High Risk Disease-modifying anti-rheumatic drugs (DMARDs) as per the guidelines in the Wyre Forest Health Partnership’s DMARD Protocol. Support site staff in the implementation and management of the DMARD process.

·       File and action blood test results from annual reviews following the Wyre Forest Health Partnership protocol.

The Wyre Forest Health Partnership (WFHP) was formed in 2014 and currently consists of 5 well established surgeries based in and around the beautiful Wyre Forest area. We have 50 GP partners and in excess of 200 staff looking after over 74,000 patients.

Do you have an interest in improving care for those with chronic kidney disease? We are looking for a clinician with a special interest in CKD to take on a care co-ordinator role across our 5 practices. We are a forward thinking Primary Care Network and have identified CKD as an area where we have the opportunity to improve and streamline care for our patients. This is an exciting new role with the ability to shape how primary care delivers care to this growing group of patients.

The role would involve proactive identification of those with CKD across our 5 practices, and providing patient information and explanation to that cohort. We would work with you to improve care pathways and enable patients with CKD to get the very best, evidence based care within our partnership, including awareness of self-management, optimisation of blood pressure management and medication- plus proactively linking in with our lifestyle advisor and social prescriber team and the secondary care renal team, to get the very best for our patients. Physicians Associate

The Wyre Forest Health Partnership (WFHP) was formed in 2014 and currently consists of 5 well established surgeries based in and around the beautiful Wyre Forest area. We have 50 GP partners and in excess of 200 staff looking after over 74,000 patients. The motivation of joining forces and forming Wyre Forest Health Partnership was to keep traditional primary care values but in a new structure that improves and protects GPs and patient care well into the future. The post holder would have a varied workload and be part of a multi-disciplinary team.

We are currently looking to recruit a Physicians Associate to work 37 hours per week (part time considered) at Bewdley Medical Centre. The successful application would participate in all treatment and preventative healthcare services in the practice as delegated and agreed by the supervising physician(s). Would also need to have knowledge of health promotion strategies, awareness of clinical governance issues in primary care and knowledge of patient group directions and associated policy.

The post holder will need to have:

·         Degree at 2:2 level or above

·         PG Dip in Physician Associate Studies from a UK PA programme

·         Current and valid certification or re-certification by the UK Physician Associate National Examination

·         Member of the Faculty of Physician Associates at the RCP, which includes registration on the UK Physician Associate Register (the PA Managed Voluntary Register)

Clinical Pharmacist

The post holder would have a varied workload and be part of a multi-disciplinary team and would be expected to work across all 5 WFHP sites.

We are looking to recruit a Clinical Pharmacist responsible for support to WFHP in implementing effective medicine management within the Partnership, identifying areas for improvement, and initiating and managing change as agreed with the senior team including:

·         Patient Services

·         Responsibility for administration

·         Promoting Clinical Excellence (or any subsequent similar CCG contract which should arise)

Extended Scope First Contact Physiotherapist Practitioner

The Partnership is looking to recruit an Extended Scope, First Contact Physiotherapist Practitioner. Reporting to our Advanced Physiotherapist Practitioner, the role will involve working autonomously, across the 5 Partnership sites, providing highly specialised assessment and treatment of patients within your area of clinical specialty. As such you will advise, influence, promote and create models of practice, which offer a continuum of care and equality of access.

This role is suitable for a Physiotherapist with enhanced skills so that they can be the first point of contact for patients with MSK symptoms. Applicants must have current UK professional registration. Experience in a similar role within Primary Care is desirable. Being qualified to carry out joint injections or willing to train is essential.

This post is a stand-alone role but you will report to the Advanced Physiotherapist Practitioner who will be able to offer support and guidance to deliver care within the partnership setting and be responsible for your own practices, be accountable for all aspects of Physiotherapy treatment and care delivered to the patient.

We are seeking to recruit Receptionists to work in our Wyre Forest Health Partnership sites.  The hours of work are 37 hours - Monday to Friday between the hours of 8.00 am to 8.00 pm with staggered starting and finishing times.

To be responsible for undertaking a wide range of reception and administrative duties which include registering patients, booking appointments, ensuring all information is managed correctly, filed and followed up accordingly. To act as the focal point of contact and be responsible for the dissemination of information, messages and enquiries for the medical team, liaising with multidisciplinary team members, and other health professionals within the practice.

We are looking for a person who is:

·         Educated to a good level of education in Mathematics and English language.

·         Experience of working in a customer focussed role would be advantageous.

Salaried GP - Stourport Medical Centre

WFHP (Wyre Forest Health Partnership) which formed six years ago and now consists of five surgeries based in the beautiful Wyre Forest area. We have 50 GPs and over staff looking after 74,000 patients.

Stourport Medical Centre is a large practice with a patient list size of approx. 21,500. We are proud to have an extensive, friendly and supportive team to include; 12 GP partners, 5 salaried GP’s, 5 ANP’s, 2 Physicians Associates, a team of 9 practice nurses, 4 HCAs, 2 clinical support pharmacists, mental health nurse, community drugs team, along with social prescribers, lifestyle advisers, management and administration staff who offer a vast range of services to our patients.  In June 2022, Stourport Medical Centre will be moving into their new, purpose state of the art facility.
